Rates

$200 per 50 minute session.

I have a limited number of sliding scale slots available.  Please free to ask about this if you otherwise would not be able to afford therapy.

Insurance

I am an out-of-network provider for all insurance plans except Medicare, for which I am in-network. Depending on your current health insurance plan, it may be possible for you to receive reimbursement for part or all of the fee you pay for psychotherapy.  I can provide you with a monthly statement to submit with your insurance claim to request reimbursement.  Please remember that you are responsible for keeping up to date on how your particular plan works.

I recommend that you contact your insurance provider directly to verify how your plan compensates you for psychotherapy services.  The following questions are good to ask to help determine your benefits:

  • Does my health insurance plan include mental health benefits?
  • If so, do I have an out-of-network benefit?
  • Do I have a deductible? If so, what is it and have I met it yet?
  • Am I responsible for paying a coinsurance, and if so, what is it?
  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
  • Do I have an out-of-pocket maximum, and if so, what is it?
  • Do I, or does my therapist, need to seek prior authorization before services can be covered?

Your insurance company representative may want to know the CPT code for the service for which you are seeking reimbursement.  The CPT code for an individual psychotherapy session is 90834.  For a couples or family session, the code is 90847.  The provider would be an out-of-network Licensed Independent Clinical Social Worker.

Payment

I accept cash, check and all major credit cards as forms of payment.  Payment is due at the time of the session, or weekly if we are meeting multiple times per week.

Cancellation Policy

A scheduled appointment means that time is reserved for you only.  If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Otherwise, you will be charged for the full rate of the session.
